Your 2024 consent form is available for download below. One player downloads and prints out the form. Every player on the team signs the form. ONE FORM FOR THE ENTIRE TEAM... .ONE FORM FOR THE ENTIRE TEAM.... This is due to me the FIRST NIGHT of volleyball. If your team does not hand this to me BEFORE you step on the sand your team gets a LOSS for that week. I will have pens and clipboards available that night but i will NOT have blank forms. You MUST bring your own form.
Individual forms will NOT be accepted. I must have ONE form for each team BEFORE you play.
